The Light-Load Isometric Protocol That Fixed My Son’s Little League Elbow in 10 Days
When my eldest son developed Little League elbow, we followed the standard advice and shut down throwing completely for two months. That shutdown stretched into three and then four months because the discomfort remained.
Searching for a better answer, I came across an interview with Dr. Keith Baar. His explanation of how tendons and ligaments actually get nutrients and adapt to stress was promising. So, I designed a simple daily protocol based on his first principles.

Results so far: My eldest son’s elbow discomfort resolved completely within about 10 days and has not returned. None of my other sons have had elbow issues since we started this protocol. We continue to use it daily.
Prior related-actity: My eldest had followed his Orthopaedic Doctor’s recommended protocol for Little League elbow for months without results. Subsequently, he followed the protocol provided by an Orthopaedic Physical Therapist for managing tendonitis for the recommended two weeks; the massage therapy component of that protocol reduced his pain and apparent internal scar tissue, but did not eliminate instances of acute pain during throwing sessions.
This is being shared because I believe it’s worth testing. And, I’m hard-pressed to see how a light-load, controlled isometric protocol could cause harm, but Caveat Emptor, consult a qualified medical professional if there is current pain or injury, yada-yada.
Our Daily Elbow Resilience Protocol
Frequency: Twice per day — once in the morning and once before bed.
Resistance: A simple rubber mallet from the tool chest (or any light object with a handle). Slide your hand toward or away from the mallet head to adjust to a comfortable level of resistance.
Nutrition Timing (we did not do this but it is recommended by Dr. Baar): 10–15g hydrolyzed collagen + 50–100mg Vitamin C taken 30–60 minutes before each session.
The Routine: Perform the four positions in sequence on one arm, then the other arm, then repeat the full sequence once more. Total time per session is roughly 8–10 minutes.
Hold Time: approx. 25–30 seconds per position.
The target “feel” is a slight, specific pull on the ligaments/tendons (different from muscle tension). You want to disengage your musculature as much as possible. My personal key word to achieve this is, “let the weight sag,” and sometimes I think, “let the bones do the work.”
You’ll usually notice the tissues relax around 20–25 seconds — hold for about 5 more seconds after that point.
The Four Positions
1. Supination at Side (Elbow at 90°)
Handle creates upward pressure on pinky and ring fingers with downward pressure on the base knuckle pad of the index finger.
2. Pronation at Side (Elbow at 90°)
Handle creates downward pressure on the interior of the thumb with upward pressure on the pinky base knuckle pad.
3. Supination at 3/4 Arm Slot
Handle creates upward pressure on pinky, ring, and middle fingers with downward pressure on the middle knuckle pad of the index finger.
4. Pronation in Deceleration Position (arm extended in front of body)
Handle creates downward pressure on the hooked index finger with upward pressure on the pinky base knuckle pad.
Final Notes
Focus on smooth, controlled tension. No jerking or sudden movements.
This is a light-load protocol designed for daily resilience and recovery. We generally do separate heavy loading work as well but did not until it appeared the injury was fully healed.
We engaged in light throwing to test if there was any discomfort. When there was discomfort in our first outing, we stopped throwing immediately. A couple days later, we did light throwing and there was no discomfort. That light throwing routine was performed until the 10th day without discomfort at which point my son felt comfortable to throw harder. We tested with a brief long toss session and continued gradually building out from there.
This approach is built on the idea that tendons and ligaments need smart mechanical loading to stay strong — not just rest. So far, the results in our house have been very encouraging.
